Disability Services Note Taking Procedures

Disability Services provides class notes as an accommodation to eligible students who have a disability that adversely affects their ability to take their own notes.  Remember, class notes provide access to the lecutre and are not a substitute for attending classes.

Students:

The following procedures have been set up to accommodate students who qualify for note taking assistance:

1.  Complete "Request for Note Taker" form and return to Disability Services. 

2.  Meet with your professor during his or her office hours or schedule an appointment in order to deliver your accommodation letter.  DO NOT GIVE THE PROFESSOR YOUR LETTER IMMEDIATELY BEFORE OR AFTER CLASS.

3.  Tell your professor you would like to make arrangements for a note taker and work out the details.  Please let your professor knwo if you want this to be handled confidentially.  You have two options:

                      a.  Request a classmae to take notes for you.  If you request your own

                           note taker, please let the  professor know that you have a note

                           taker.

                       b.  Request a volunteer note taker.  Ask your professor to pass out the

                           "Request for Volunteer Note Taker" form in class.  The professor

                            should pass out the for the first week of class and return the form to

                            Disability Services as soon as possible.  Once, we receive the

                            form, we will contact the volunteer with additional information.

You are strongly encouraged to continue to take your own notes too, as note taking aids comprehension and retention.  The second set of notes should be available at the end of each class or the following day. 

Arrange with the professor if youw ant the notes delivered to you, to Disability Support Services, or the professor.  It will be your responsibility to make appropriate arrangements. 

Please be advised that you are expected to be conscientious in your attendance.  The professor may ask the note taker to stop sharing notes if absences are excessive (unless excused due to medical reasons).  To resume note taking, you must see the professor and the Disability Services Coordinator.

If you have any problems with theses procedures which cannot be resolved with the professor or with the note taker, please contact the Disability Services Coordinator at: 594-8763. 

Professors:

Students who qualify for note taking accommodations should present necessary documentation identifying their specific needs to the Disability Services Coordinator. After meeting with the coordinator, the student will deliver a letter of accommodation stating they qualify for note taking assistance.  The student has been instructed to make an appointment to see you during office hours, NOT before or after class.  The student should inform you if they would like note taking to be handled confidnetially.  The student has two options, they can ask a classmate to take notes or the student can request a volunteer to take notes.  If the student would like a volunteer, please pass out the "Request for Volunteer Note Taker" form as soon as possible.  Once it is completed, please return it to the Academic Advising Center (DSU 3125).  The office staff will then contact the volunteer and make sure the notes are brought in for the student.  The student is advised that attendance is required and note taking assistance is not a substitute for class attendance.  If a student is not attending class and absences are excessive, you have the option to ask the note taker to stop sharing notes (unless the abscence is excused due to medical reasons).  Please contact the Disability Services Coordinator if a students absences become excessive.  The student will have to meet with the Disability Services Coordinator to resume note taking.  If you have any problems with these procedures, please do not hesitate to contact the Disability Services Coordinator in the Academic Advising Center (DSU 3125) or call (757) 594-8763.
